About the Opportunity
The Homelessness Access Program will aim to increase homeless young people’s access to the primary care services by eliminating barriers that restrict young people’s involvement, increasing their opportunity to engage in longer-term care and support.
This position will create a conduit between young people experiencing or at risk of experiencing homelessness and Primary care settings. Working collaboratively as part of a young person’s care team in conjunction with homeless services, General Practitioner’s (GPs), existing Community Nurses and other allied health staff, this position will provide nursing, health promotion and counselling for young people aged 16 to 25 years across the Barwon and Colac areas.
The key objectives of this role are:
- Provide assertive outreach to young people in Housing and homelessness settings; with a focus on those accessing Youth Homelessness programs and our emergency and crisis accommodation.
- Build on the existing partnerships between Meli and Headspace Geelong, offering centre-based support at both Meli youth specific locations and the hG sites.
- Build on existing partnerships with NEAMI and the Salvation Army to improve health outcomes for young people at risk of or experiencing homelessness.
- Provide intake, assessment and primary physical health support to young people including sexual health counselling.

About You
To be successful in the role, you will have:
- Tertiary qualifications and current registration in Nursing (Registered Nurse) with completed Nurse Vaccination Training, Sexual and Reproductive Health Training, Mental Health First Aid and/or ASSIST Training
- Demonstrated experience in working with young people in a homelessness and/or primary health care setting.
- Comprehensive and practical understanding and ability to manage complex health presentations with psychosocial issues and including moderate to acute mental health and or Alcohol and Other Drug issues.
- Ability to understand and support clients via assessment to manage supports, connections and risk associated with homelessness and/or family violence.
