We are currently seeking a compassionate and skilled Child and Family Disability Practitioner to provide vital support to families, helping them access the services they need and enhance their capacity to care for children with disabilities.

About the Role

As a Child and Family Disability Practitioner, you will:
• Provide case management to families, enhancing their capacity to care for children and young people.
• Coordinate supports to ensure children remain safely in their family homes with the necessary mainstream and disability services in place.
• Deliver a range of tailored interventions in line with the Best Interests Framework to support the targeted cohort.
• Assist families in accessing and implementing the services and supports outlined in their NDIS plans.
• Collaborate with a passionate and professional team dedicated to providing high-quality, family-centered support.

Position Details

  • Fixed term position until 30 June 2025
  • 1 FTE working 38 hours per week
  • $97,614 p/a + Super + Salary Packaging

What We're Looking For

We value individuals who bring dedication, empathy, and expertise to their roles. To succeed in this position, you will have:

  • Qualification in Social Work, Psychology, Nursing, Diploma of Welfare Studies, Diploma of Alcohol and Other Drugs or related field
  • Proven ability to manage complex cases, including client advocacy, counselling, therapeutic interventions, and family support, in line with the Best Interest Framework.
  • Strong skills in risk and needs assessments, with knowledge of factors impacting parental capacity and available services to support children, parents, and young people.
  • Understanding of the Victorian Child Safe Standards, with a demonstrated commitment to fostering a child-safe organisation in practice and culture.
  • Victorian Driver’s Licence
